613 Physio (operating as K. Hidalgo Physiotherapy Professional Corporation) is a mobile physiotherapy clinic serving Ottawa and surrounding communities including Greely, Barrhaven, Manotick, Kanata, Riverside South, Orleans, and other areas across the region. Our team of registered physiotherapists, kinesiologists, and rehabilitation specialists provides in-home, workplace, recreational, and virtual physiotherapy services.

We are committed to protecting your privacy and the confidentiality of your personal and health information. This Privacy Policy describes how we collect, use, store, and share your information in connection with all aspects of our business operations, including our website, patient portal, appointment booking, clinical services, and any digital health integrations.

This policy is prepared in compliance with the Personal Information Protection and Electronic Documents Act (PIPEDA), applicable Ontario privacy legislation, and the standards of the College of Physiotherapists of Ontario.

2. Scope of This Policy

This Privacy Policy applies to all personal information collected by 613 Physio through:

  • Our patient portal and booking system powered by Jane App (613physio.janeapp.com)
  • In-person clinical services delivered at your home, workplace, or recreational space
  • Virtual physiotherapy sessions conducted online
  • Phone and email consultations and communications
  • Digital health integrations, including the Garmin Health API
  • Our social media channels (Instagram and Facebook)
  • Payment processing and insurance billing
  • Job applications and employment inquiries

 

This policy applies to patients, prospective patients, website visitors, job applicants, and any other individuals whose personal information we handle.

3. Information We Collect

3.1 Patient and Clinical Information

To provide physiotherapy and rehabilitation services, we collect:

  • Full name, date of birth, sex, and contact information (address, phone, email)
  • Health history, medical history, and records of past injuries or conditions
  • Physiotherapy assessment findings and treatment notes
  • Exercise programs, treatment plans, and progress records
  • Referral information from physicians or other healthcare providers
  • Consent forms and signed treatment agreements

 

3.2 Booking and Administrative Information

When you book appointments or contact us, we collect:

  • Appointment scheduling data through Jane App
  • Communications via email, phone, or our website contact form
  • Insurance provider details and policy numbers
  • Payment records including credit card transactions and e-transfer confirmations
  • Billing receipts and insurance claim documentation

 

3.3 Website and Digital Information

When you visit our website, we may automatically collect:

  • Browser type, device type, and operating system
  • IP address and approximate geographic location
  • Pages visited, time spent on pages, and referring URLs
  • Information submitted through contact or consultation request forms (name, email, message)

 

3.4 Digital Health and Wearable Device Data (Garmin Health API)

With your explicit, informed consent, we may collect health and fitness data from your Garmin device or Garmin Connect account through the Garmin Health API to support your physiotherapy care. This may include:

  • Physical activity data (steps, distance, active minutes, calories burned)
  • Heart rate, resting heart rate, and heart rate variability (HRV)
  • Sleep data (duration, sleep stages, quality scores)
  • Stress level indicators and body battery readings
  • Oxygen saturation (SpO2) readings
  • Movement, gait, and mobility metrics relevant to rehabilitation assessment

Garmin data is collected only when you provide explicit consent and is used exclusively for clinical and therapeutic purposes directly related to your care.

 

3.5 Employment and Contractor Information

If you apply for a position with 613 Physio, we may collect your resume, cover letter, credentials, references, and any other information you voluntarily provide during the application and hiring process.

4. How We Use Your Information

We use the information we collect for the following purposes:

 

4.1 Providing Clinical Services

  • Delivering physiotherapy, kinesiology, and rehabilitation treatment
  • Conducting initial assessments and building personalized treatment plans
  • Monitoring your recovery progress using clinical and, where consented, wearable device data
  • Providing virtual physiotherapy sessions
  • Referring you to other healthcare providers when appropriate

 

4.2 Booking and Administration

  • Scheduling and managing appointments through Jane App
  • Sending appointment reminders and follow-up communications
  • Processing payments and issuing receipts
  • Processing insurance claims and reimbursement requests with your authorization

 

4.3 Communication

  • Responding to inquiries submitted via our website, email, or phone
  • Sending clinical updates, exercise instructions, or treatment-related messages
  • Notifying you of changes to our services or this Privacy Policy

 

4.4 Website and Business Improvement

  • Analyzing website traffic and usage to improve our online presence
  • Understanding patient needs to improve our services
  • Legal compliance and record-keeping obligations

 

4.5 Employment

  • Evaluating job applications and communicating with candidates
  • Onboarding and managing employment relationships

We will never use your personal or health information for advertising profiling, targeted marketing, or any purpose unrelated to your care or our legitimate business operations without your explicit consent.

5. Jane App (Patient Management System)

We use Jane App, a Canadian-based healthcare practice management platform, to manage patient records, appointment scheduling, billing, and our online patient portal. Jane App stores and processes patient information on our behalf as a trusted service provider.

Jane App is compliant with Canadian privacy legislation including PIPEDA and stores data on Canadian servers. You can access your patient portal at 613physio.janeapp.com to view your appointments and records. Jane App’s own privacy policy is available at janeapp.com.

By using our services, you acknowledge that certain information will be stored and processed through Jane App’s platform. We have a data processing agreement in place with Jane App to protect your information.

6. Garmin Health API Integration

Where you choose to connect your Garmin device or Garmin Connect account to support your care, the following applies:

  • Connection to Garmin is entirely voluntary and requires your explicit informed consent
  • You may revoke access to your Garmin data at any time through your Garmin Connect account or by contacting us
  • Garmin health data is used exclusively by your treating clinician to inform and personalize your treatment plan
  • Garmin data is accessible only to authorized 613 Physio clinical staff involved in your care
  • We do not share your Garmin health data with third parties except as required by law or described in Section 7 of this policy
  • Revoking Garmin access does not affect the continuation of your physiotherapy services

7. Disclosure of Your Information

We do not sell, rent, or trade your personal or health information. We may share information only in the following circumstances:

 

7.1 With Your Consent

We will share your information with other parties only when you have provided clear, informed consent to do so. For example, sharing records with your family physician or another specialist at your request.

 

7.2 Healthcare Providers Involved in Your Care

We may share relevant clinical information with other registered healthcare professionals directly involved in your treatment (e.g., your referring physician, specialist, or other allied health provider) to ensure continuity and quality of care.

 

7.3 Insurance Providers

With your authorization, we may share treatment records and billing information with your insurance provider (e.g., Canada Life, Sun Life, Great-West Life, Blue Cross, DVA) to process claims and reimbursements.

 

7.4 Service Providers

We work with trusted third-party service providers who assist us in operating our business, including Jane App (practice management), payment processors, and website hosting. These providers are bound by confidentiality agreements and are only permitted to use your information as necessary to provide services to us.

 

7.5 Legal Requirements

We may disclose your information when required by law, court order, or regulatory authority, or when we believe disclosure is necessary to prevent harm, protect safety, or comply with a legal obligation under applicable Canadian law.

 

7.6 College of Physiotherapists of Ontario

As registrants of the College of Physiotherapists of Ontario, our clinicians are subject to regulatory oversight. In the event of a complaint or audit, relevant records may be disclosed to the College as required.

8. Payments and Insurance

613 Physio accepts Visa, Mastercard, and e-transfer. Payment is collected at the conclusion of each session. Receipts are issued that are accepted by most extended health benefit plans.

We do not currently offer direct billing. Patients are responsible for submitting receipts to their insurance provider for reimbursement. We do not accept WSIB or motor vehicle accident (MVA) cases.

Payment card data is processed through secure, PCI-compliant payment processing systems. We do not store your full credit card numbers.

9. Website and Cookies

Our website (613physio.com / 613physio.ca) may use cookies and similar tracking technologies to improve your experience and understand how visitors use our site. Cookies are small text files stored on your device.

We may use the following types of cookies:

  • Essential cookies: Required for the website to function correctly
  • Analytics cookies: Help us understand how visitors interact with our site (e.g., pages visited, session duration)

You may disable cookies through your browser settings. Note that disabling certain cookies may impact the functionality of our website or booking system. We do not use cookies to collect clinical or health data.

Our website may contain links to third-party sites (e.g., Jane App patient portal, social media). This Privacy Policy does not apply to those external sites. We encourage you to review their privacy policies.

10. Social Media

613 Physio maintains accounts on Instagram and Facebook. If you interact with us through these platforms (e.g., by messaging, commenting, or following our accounts), please be aware that those interactions are subject to the privacy policies of those respective platforms.

We do not collect clinical or sensitive health information through social media channels. Please use our secure contact methods (email or phone) to share any personal or health-related information.

We will never publicly share identifiable patient information through social media without explicit written consent.

11. Virtual Physiotherapy Services

613 Physio offers virtual physiotherapy sessions for patients who prefer remote care. Virtual sessions are conducted through secure, privacy-compliant video conferencing platforms.

During virtual sessions:

  • Session content is not recorded without your explicit consent
  • You are responsible for ensuring you are in a private location during your session
  • Clinical notes from virtual sessions are stored in your Jane App patient record in the same manner as in-person sessions

12. Data Retention

We retain your personal and health records for a minimum of ten (10) years following your last treatment, or as required by applicable Ontario health legislation and College of Physiotherapists of Ontario standards.

Garmin health data collected through our integration is retained for the duration of your active treatment relationship with us and for a reasonable period thereafter to support continuity of care. You may request deletion of your Garmin-sourced data at any time, subject to our obligations to retain clinical records.

Website analytics data is retained in aggregate, anonymized form. Contact form submissions are retained for the period necessary to address your inquiry.

Job applicant information is retained for six (6) months following a completed hiring process, after which it is securely deleted unless you consent to it being kept longer for future opportunities.

13. Data Security

We take the protection of your information seriously and implement the following safeguards:

  • Encrypted data storage and transmission (TLS/SSL)
  • Secure, access-controlled practice management systems through Jane App
  • Role-based access controls limiting data access to authorized clinical and administrative staff
  • Secure payment processing through PCI-compliant systems
  • Staff training on privacy obligations and data handling practices
  • Regular review of our security practices and technology

 

No system is completely secure. In the event of a privacy breach that poses a real risk of significant harm, we will notify affected individuals and report to the Office of the Privacy Commissioner of Canada as required under PIPEDA.

14. Your Privacy Rights

Under PIPEDA, you have the following rights regarding your personal information:

  • Right to Access: You may request a copy of the personal information we hold about you
  • Right to Correction: You may request correction of inaccurate or incomplete information
  • Right to Withdraw Consent: You may withdraw consent to certain uses of your information at any time, subject to legal or professional obligations
  • Right to Know: You may ask how your information is being used and with whom it has been shared
  • Right to Complain: You may file a complaint with the Office of the Privacy Commissioner of Canada if you believe your privacy rights have been violated

 

To exercise any of these rights, please contact our Privacy Officer using the details in Section 15. We will respond to all privacy requests within thirty (30) days.
